By accessing or using the Wellness Eligibility Program website (“Site”), you agree to be bound by these Terms and Conditions. If you do not agree with any part of these terms, you must not use our Site.
The Wellness Eligibility Program (“WEP”) is a marketing initiative that connects eligible Medicare beneficiaries with licensed physicians who may order genetic screening when medically appropriate. WEP is not a healthcare provider, insurance company, or laboratory. WEP is not affiliated with or endorsed by Medicare, the Centers for Medicare & Medicaid Services (CMS), or any government agency.
Information submitted through our eligibility forms is used solely to determine potential qualification for Medicare-covered genetic screening programs. Submission of information does not guarantee eligibility, coverage, or that a genetic test will be ordered. All testing decisions are made by licensed physicians based on medical appropriateness.
Medicare Part B may cover certain genetic tests when ordered by a licensed provider and determined to be medically appropriate. Coverage depends on your specific Medicare plan, eligibility, and the physician’s clinical determination. WEP does not make coverage determinations. For questions about your specific Medicare benefits, contact Medicare directly at 1-800-MEDICARE (1-800-633-4227).
The content on this Site is for informational purposes only and does not constitute medical advice, diagnosis, or treatment. Always consult with a qualified healthcare provider regarding any medical questions or conditions. The information provided through our screening programs is intended to supplement, not replace, the relationship between a patient and their healthcare provider.
By submitting your information through our forms, you provide Express Written Consent authorizing the Wellness Eligibility Program Team to contact you via voice, SMS, or email at the information provided. You understand that:
You agree to provide accurate, current, and complete information when using our eligibility forms. Providing false or misleading information may result in denial of services and may violate federal and state laws.

To the fullest extent permitted by law, WEP shall not be liable for any indirect, incidental, special, consequential, or punitive damages, or any loss of profits or revenues, whether incurred directly or indirectly, arising from your use of the Site or our services. WEP’s total liability shall not exceed the amount you paid to WEP, if any, in the twelve (12) months prior to the claim.
You agree to indemnify and hold harmless WEP, its officers, directors, employees, and agents from any claims, damages, or expenses arising from your use of the Site, violation of these Terms, or infringement of any rights of another party.
These Terms and Conditions shall be governed by and construed in accordance with the laws of the State of Florida, without regard to its conflict of law provisions.
